What is American Toad-in-a-hole? Is it different from UK - I guess it must be as the UK version doesn't use bread.

Yes. An egg is cooked in butter in a hole in a slice of bread, a waffle or bagel. Make a hole in a slice of bread with the rim of a glass or cookie cutter about 2-3". Crack an egg in the hole, and cook in butter in a skillet on the stove top. I've seen several eggs baked in several cut holes, in a loaf of Italian or French bread. Aka eggs in a basket.
