How to minimize the carbohydrates in daily eating?

OK, I've been reading this really interesting book called Why We Get Fat: And What To Do About It (by Gary Taubes) and according to him, it's not so much about the calories that we intake, rather than how much carbohydrates we consume (so basically, the carbs are completely at fault for us being fat, nothing else determines it).

That does somewhat make sense to me, but would you go about minimizing carbs and implementing a sustainable plan with it? Bread, pasta, potatoes (oh man, potatoes too?), rice, sugar (chocolate, honey)... how could one possibly get rid of all of these, if he's already so used to eating them? Can you break the habit, like trying to quit smoking?

Any ideas are welcome.
